3.\u00a0 Enter your Employee Number by using your Social Security Number ( 9 digits without the dashes)<\/p>\n\n\n\n
4.\u00a0 Enter the Personal Identification Number that is comprised of the Last Four Digits of Your Social Security Number and Your two Digit Year of Birth
